PRODUCT & SERVICES POLICIES

These policies describe how Firefly Medical Group PLLC (“FMG,” “we,” “us”) provides services and products. By engaging with FMG, you agree to these policies and to the Terms & Conditions you will be asked to sign before treatment.

General Conditions

  • FMG may refuse service to anyone for any lawful reason.
  • Headings are for convenience only.
  • This Agreement is governed by North Dakota law.
  • FMG’s failure to enforce any right or provision does not waive it.
  • If any part of this Agreement is found unenforceable, the remainder remains valid.

Payment & Financial Policy

  • FMG does not accept insurance. All services and products are self-pay.
  • Fees are due at the time of service unless otherwise arranged.
  • Packages may require deposits or commitment periods.
  • All sales are final. Refunds are not provided except as required by law or if FMG made a billing error.
  • Specific details, including late fees, interest, and failed payment charges, are explained in FMG’s Financial Responsibility form and signed Terms & Conditions.

Appointments, Cancellations & No-Shows

  • Appointments are subject to availability and may be rescheduled by FMG.
  • Appointment cancellations require 24-hour notice.
  • Late cancellation or no-show fees apply and are set out in the signed Terms & Conditions.

Products & Services

  • Services and products may be limited in quantity or availability.
  • Descriptions, prices, and offerings may change at any time without notice.
  • FMG does not guarantee results or effectiveness of any service or product.

Conduct 

  • Clients are expected to treat staff, other clients, and visitors with respect. 
  • Disruptive, abusive, or unsafe behavior will not be tolerated and may result in denial or termination of services. 
  • Prohibited conduct includes: 
    • Providing false or misleading information 
    • Unauthorized recording of staff or providers 
    • Harassment, threats, or unsafe actions 
    • Uploading malware or misusing FMG systems 
    • Infringement of FMG’s intellectual property

Privacy

  • Your personal information is protected under FMG’s Privacy Policy and HIPAA Notice of Privacy Practices.

Termination of Services

  • Either you or FMG may terminate services, subject to the signed Terms & Conditions and the signed terms relating to the service.
  • Outstanding balances are due immediately upon termination.
  • No refunds or prorated amounts are provided for services or products already delivered.

Changes to These Policies

  • FMG may update these policies from time to time. Your continued use of FMG’s services constitutes acceptance of any updates.

Indemnification

By using FMG’s services, you agree to indemnify and hold FMG, its owners, members, officers, directors, agents, contractors, service providers, partners, licensors, insurers, employees, and assigns harmless from any claims, damages, or costs (including attorney fees) arising from your actions, except where caused solely by FMG’s willful misconduct or gross negligence.
